5-Point Performance Rating Scale
Outstanding (5) – Exceeds Expectations
• Consistently performs at an exceptional level, going above and beyond job requirements.
• Demonstrates initiative, innovation, and leadership in responsibilities.
• Requires little to no supervision and often assists others in improving their performance.
Exceeds Expectations (4) – Above Average
• Regularly performs above the expected level of competence.
• Demonstrates strong skills, reliability, and initiative in their work.
• Occasionally exceeds job expectations and contributes to team success.
Meets Expectations (3) – Satisfactory
• Performs job duties at the expected level with consistency.
• Demonstrates competence and reliability in daily tasks.
• Requires normal supervision and meets established goals.
Needs Improvement (2) – Below Expectations
• Performance is inconsistent or does not fully meet expectations.
• Requires additional supervision, guidance, or training.
• May struggle with some aspects of the job and needs improvement in key areas.
Unsatisfactory (1) – Does Not Meet Expectations
• Fails to meet job requirements on a regular basis.
• Requires frequent supervision and corrective action.
• Performance negatively impacts individuals served, team, or organization.
